Common Housing Forms


Housing Application Cancellation Form: Students wish to cancel their submitted housing application may do so via the link provided.

Early Move In Request Form: Students wishing to arrive and move in prior to their scheduled move in date may submit a request form via the provided link. Early arrivals may not extend more than 10 days prior to the contract start date.

Late Move In Request Form: Students who need to arrive later than the available move in appointment times should let us know via the Late Arrival Request Form linked above.

Late Move Out Request Form: Students who need an extension on their move out time at the end of term should submit the late departure request form linked above. Late stays cannot exceed 3 days in length past the contract end date.

Maintenance Request Form: Students who need service performed in their residence hall room or apartment should utilize the link below to submit a maintenance request form. Students will need to utilize the campus network, or extablish an off campus UCCS VPN Connection, to submit a maintenance request form.

Incident Report Form: If you need to report an incident that has occured in housing that involves student behavior or conduct, utilize the form linked above. Questions about this form can be directed to Molly Kinne at mkinne2@uccs.edu.
